Landowner Signatures and Agent Designation Form - All owners of each parcel must sign. Original signatures are required. All signatures must be dated within the first six months of the first signature obtained.
Disclosure of Gifts to LAFCO Commissioners
LAFCO Fee Schedule - Initial fee deposit equal to half of LAFCO fees are due at the time an application is submitted. The second deposit is due with the final application requirements. LAFCO will bill until the project is closed.
Preliminary Descriptive Map (8½" or 11" x 17" format)
Service Plan: Applicant Guidelines, LAFCO Policies
Feasibility Study, if applicable
FINAL APPLICATION REQUIREMENTS - due before LAFCO hearing:
Environmental Review: CEQA Requirements, Fish and Game Fees
